TMJ Dysfunction: How Jaw Pain Connects to Neck Pain and Headaches

What Is TMJ Dysfunction?

Temporomandibular joint dysfunction (TMD) is a group of conditions producing pain and functional problems at the jaw joint β€” just in front of the ear. The TMJ is one of the most complex joints in the body, combining rotation and translation in a coordinated movement that opens, closes, and moves the jaw. When this mechanism breaks down, the resulting pain can significantly impact quality of life.

At Pittsburgh Physical Medicine, we see TMD patients from across Shadyside, Lawrenceville, Bloomfield, Squirrel Hill, Oakland, Highland Park, Point Breeze, and Regent Square who have often been bouncing between dentists, oral surgeons, and neurologists without lasting relief β€” because the cervical spine component is consistently missed.

The TMJ-Neck-Headache Connection

The muscles controlling jaw movement share neural pathways with the upper cervical musculature through the trigeminal-cervical nucleus in the brainstem. This means cervical joint dysfunction can directly sensitize the TMJ pain system β€” and TMJ dysfunction can refer pain into the neck and head, mimicking tension headaches. Treating only the jaw without addressing the cervical spine often produces incomplete results.

Causes and Contributing Factors

Symptoms

Symptom overlap: TMD frequently mimics earaches, tension headaches, trigeminal neuralgia, and cervicogenic headaches. If you've been treated for these without lasting relief, TMD should be evaluated as a contributing factor.

How We Treat TMJ Dysfunction

Cervical Chiropractic Care

Restoring upper cervical joint mobility β€” particularly at C1-C2 β€” directly reduces the facilitated pain processing that amplifies TMD symptoms. This is often the missing piece when dental treatment alone has failed.

Soft Tissue Release

Trigger point release of the masseter, temporalis, and pterygoid muscles reduces the muscular component of jaw pain significantly.

Physical Therapy

Dr. Crockatt provides jaw mobility exercises, postural correction, and daytime jaw habit education. We coordinate with dentists and oral surgeons when nightguard therapy or specialist evaluation is warranted.
